Description: | The use of nickel-titanium (NiTi) files during biomechanical root canal preparation has been associated with the formation of dentinal microcrack. The present study aimed to evaluate and compare the presence of microcrack and its severity following root canal preparation with continuous rotation full-sequence or reciprocating singlefile system. Forty extracted human mandibular premolars having single canal and straight root were randomly assigned to four experimental groups (n=10): ProTaper Next (PTN), ProTaper Gold (PTG), Reciproc Blue (RCPB) and WaveOne Gold (WOG). The samples were scanned using high resolution micro-computed tomographic imaging before and after root canal preparation. Samples were prepared to file size #25. Then, pre-instrumentation and post-instrumentation scanned images were analyzed by two blinded observers for the presence and extent of dentinal defects. For each group, the number of slices with microcracks was calculated as a percentage rate. Data were analyzed using McNemar and Fisher's Exact tests at a significance level of p < 0.05. A total of 113,000 cross-sectional images were examined. In these images, 1,105 (1.00%) dentinal defects were seen. Dentinal microcracks were seen in 0.2% (n=233), 0.2% (n=250), 0.3% (n=239), and 0.3% (n=293) of cross-sectional images of PTG, PTN, WOG, and RCPB groups respectively. For all the groups, the number of microcracks identified in the postinstrumentation analysis stage were already present in the corresponding preinstrumentation analysis stage. The types of dentinal defects propagated by different kinematic motions and sequences of rotary instruments were not statistically significant p = 1.000. Within the limitations of this study, biomechanical preparation with different types of tested engine-driven files did not induce formation of new microcracks at different root third levels.
