Description: Cleft lip and palate (CLP) patients are managed by health care professionals from multiple disciplines following protocols since birth. One of the protocols includes providing oral healthcare. Despite the existing protocols, the oral health status (OHS) of CLP patients remains a concern. Caregivers’ play an important role in CLP children’s OHS. Hence, the factors associated with caregivers needs to be further investigated. This study was carried out to determine the association between the caregivers’ factors, including their oral health literacy (OHL) level with their CLP children’s OHS. This is a multi-centred study involving 113 pairs of caregivers and their CLP children aged between two to twelve years old. The caregivers’ factors and OHL level of the caregivers were assessed using a set of a self-administered questionnaire in Malay version of 14-item Health Literacy in Dentistry (M-HeLD-14). Clinical examination was conducted to determine the children’s dental caries by using WHO’s dft/DMFT index, and oral hygiene status was evaluated by using Greene and Vermillion’s OHI-S index. Data analysis was conducted by using IBM SPSS version with the level of significance set at p<0.05. Correlational analysis was conducted to look at the relationships between continuous variables, and Chi-square analysis was used to look at the differences between categorical variables. Binary logistic regressions were performed to identify caregivers’ factors that influence the CLP children’s OHS. The mean age of children, when first seen by the dental team was 26.46 ± 23.16 months old. 73.5% of the CLP children had dental caries experience in their deciduous teeth, while 21.2% had caries experience in their permanent teeth. A significant negative correlation was noted between the OHL level of caregivers and their CLP children’s untreated deciduous caries (dt)(r=-0.565, p<0.001), deciduous caries experience (dft)(r=-0.513, p<0.001), and oral hygiene (OHI-S)(r=-0.538, p<0.001). Caregivers with household income within the bottom 40%, with education level that is less than a diploma, and having inadequate OHL had CLP children with poorer OHS (p<0.05). Caregivers’ OHL remained to be a contributing factor for CLP children’s deciduous caries and oral hygiene status according to the logistic regression analysis (p<0.01). In conclusion, besides having low economic status and low educational level, caregivers’ OHL is an important contributing factor to CLP children’s OHS.
