dc.descriptionNeck pain (NP) is a painful musculoskeletal condition that is frequently managed using physiotherapy. However, physiotherapy services are not readily available in all districts. One approach that can provide a partial respite from this problem is telerehabilitation. The objectives of this study were to: develop a physiotherapy based telerehabilitation system (telePTsys), determine its validity and reliability for assessment of NP, examine the clinical feasibility of the system and explore patient perceptions and satisfaction. In Phase 1 of the study, the main structure, content and system design for the development of the telePTsys were identified based on focus group discussions. Phase 2 addressed the validity and reliability of the telePTsys. Eleven adults (mean age: 32.7 ± 10.9 years) with non-specific (NS)-NP were recruited and assessed for physical outcome measures using the telePTsys and face-to-face methods. In Phase 3, a quasi-experimental pre- and post-intervention design study was conducted in 10 adults (mean age: 33.4 ± 14.7 years) with NS-NP. Duration of the intervention was four weeks. Baseline outcome measures were compared in second and fourth weeks. Adults with NS-NP were provided pain education, postural corrections, exercises, a reference website for neck exercise videoclips with an e-exercise logbook based on suggestions from Phase 1 study results. Patient satisfaction was evaluated using Telemedicine Satisfaction Questionnaire at the end of fourth week. Phase 2 results showed a high degree of concurrent validity for pain intensity (bias = 0.90), posture (bias = 0.96o), deep neck flexor (DNF) endurance (bias = -2.3 seconds), disability index (bias = 0.10), active range of motion (AROM) (bias = 0.30 to -1.00). A high degree of inter- (ICC 3,1 = 0.96 to 0.99) and intra-rater reliability (ICC3,1 = 0.93 to 0.99) was achieved for pain intensity, posture, AROM, DNF endurance and disability index measurements. There was significant agreement for combination movements for validity, and inter- and intra-rater reliabilities (76.4% - 78.5% of agreement, p < 0.05). The findings of Phase 3 revealed significant (p < 0.001) decrement in pain intensity and disability index with increased in DNF endurance, and AROM of extension and rotation following four weeks of intervention via telePTsys in adults with NS-NP. Despite high levels of satisfaction towards telePTsys and service model comparative adherence to home-based exercise therapy was moderate. The study results show that the telePTsys is feasible, valid and reliable suggesting that there may a possibility to extend telePTsys services as a remote rehabilitation service for patients with NS-NP in the near future.,Ijazah Doktor Falsafah-
