dc.descriptionAge-related decline in mental health and cognitive function usually starts to show its sign during middle-age and is greatly affected by dietary factors. Polyphenols have recently received attention in its role to improve various health issues related to aging, including decline in mental health and cognitive status. Therefore, the present study aims to determine the prevalence of poor mental health and cognitive status among middle-aged adults and its risk factors. Furthermore, the study aimed to determine the effects of polyphenols-rich TP 3-in-1™ juice in improving mental health, cognitive status, selected biochemical parameters and metabolomics profiles among middle-aged adults. This study was conducted in two phases. Phase 1 was a cross-sectional study to determine the prevalence of poor mental health and cognitive status using various neuropsychological tests involving 349 (65.3% women, mean age 52.5 ± 4.2 years) middle-aged adults, recruited from low-cost residential flats around Zone 1 Cheras. Phase 2 involved a clinical trial among 31 middle-aged women with signs of poor mental health and cognitive status, who were randomised to receive either supplementation of polyphenols-rich TP 3-in-1™ juice (n=16) or placebo (n=15). Supplementation was given for three days in a week (three times in the day of beverage consumption) for a period of ten weeks. Result of Phase 1 indicated that more than 40% of middle-aged adults were identified as having signs of poor mental health according to total General Health Questionnaire (GHQ) score. Furthermore, a total of 67.9% of the subjects had poor cognitive status according to Rey's Auditory Verbal Learning Test (RAVLT) immediate recall score. Hierarchical binary logistic regression indicated that fat intake was found to be associated with somatic symptoms for both men (adjusted OR=1.04; p<0.05) and women (adjusted OR=1.06; p<0.05). Intake of iron was associated with lower GHQ score among women (adjusted OR=0.689; p<0.05). Intake of lignan (adjusted OR=1.071; p<0.05) was found to be associated with better RAVLT immediate recall among women. Additionally, high cholesterol (adjusted OR=3.14; p<0.05) was found to be associated with poor score of RAVLT delayed recall for women. For Phase 2, there were significant interaction effects on RAVLT immediate recall (p<0.05) and Comprehensive Trail Making Test (CTMT) Trail 4 (p<0.05). Metabolomics analysis showed the presence of metabolites related to polyphenols intake and cognitive functions with the intervention group showed increased urinary excretion of thyroxine and 3-methyladenine. Thyroxine and 3-methyladenine give stability to human transthyretin (TTR) and activates autophagy, respectively, which are associated with the pathogenesis of Alzheimer's disease. Early detection of poor mental health and cognitive status is crucial to prevent further decline among the middle-aged adults, which may leads to Alzheimer's disease in old age. Supplementation of TP 3-in-1™ juice which is rich in anthocyanins for 10 weeks has the potential to improve cognitive status related to learning, memory, processing speed, sequencing, mental flexibility and visual-motor skills among middle-aged women.,Ijazah Doktor Falsafah (Dietetik)-
