dc.description | Kajian lepas di seluruh dunia terutamanya di Barat menyarankan gaya interaksi negatif dalam bentuk indeks ekspresi emosi tinggi (EE-tinggi) serta komen berbentuk kritikan (KBK-tinggi) dan sikap pencerobohan emosi (PE-tinggi) yang diluahkan oleh keluarga terhadap penghidap skizofrenia adalah peramal psikososial yang kuat kepada pengulangan semula episod psikotiknya. Namun, fenomena ini masih tidak diketahui di Malaysia. Kajian ini bertujuan untuk menyelidik: (i) profil penghidapan dan pengulangan semula skizofrenia; (ii) profil indeks EE dan komponen KBK serta PE dalam kalangan ahli keluarga; (iii) profil tret personaliti serta atribusi yang dipegangi ahli keluarga dalam menjelaskan tentang simptomatologi yang dialami penghidap; (iv) mekanisme yang mendasari pembentukan indeks EE serta komponen KBK dan PE ahli keluarga; dan (v) mekanisme yang mendasari pengulangan semula skizofrenia. Seramai 160 responden (N=80 penghidap skizofrenia kronik dan N=80 ahli keluarga mereka) terlibat dalam kajian ini. Penghidap yang sedang menerima rawatan penghospitalan di wad psikiatri, memenuhi kriteria penyertaan dan penyingkiran serta mendapat keizinan daripada keluarga ditemu bual untuk penaksiran simptomatologi mereka (Fasa 1) menggunakan alat ujian Structured Clinical Interview for Positive and Negative Syndrome Scale (SCI-PANSS). Penghidap juga terlibat dalam kajian susulan 6 bulan kemudian untuk dinilai kemungkinan berlakunya pengulangan semula skizofrenia (Fasa 2). Indeks EE dan komponennya serta tret personaliti dan atribusi keluarga diukur menggunakan Family Questionnaire (FQ), Eysenck Personality Questionnaire-Revised versi pendek semakan (EPQ-RS) dan Attributions of Symptoms Inventory (ASI). Ujian statistik yang digunakan adalah ujian-t satu sampel, ujian-t sampel bebas, analisis jadual kontingensi dua hala berasaskan crosstabs, statistik Khi-kuasa dua Pearson, analisis regresi pelbagai, regresi logistik dan analisis laluan. Keputusan bagi profil penghidapan skizofrenia mendapati penghidap memperlihatkan simptom positif, simptom negatif dan keseluruhan simptomatologi di tahap yang parah secara klinikal. Seramai 52.5% penghidap mendapat pengulangan semula dalam tempoh 6 bulan kajian susulan. Kes pengulangan semula adalah lebih ramai secara signifikan dalam kalangan penghidap yang tinggal bersama ahli keluarga berindeks EE-tinggi dan dengan tahap KBK-tinggi. Tahap keparahan simptom positif adalah penyumbang utama kepada komponen KBK-tinggi, manakala tret kebolehinginan sosial adalah penyumbang utama kepada komponen PE-tinggi. Tret ekstraversi dan neurotikisme keluarga berserta simptom positif penghidap adalah peramal yang signifikan kepada indeks EE-tinggi. Indeks EE-tinggi keluarga adalah peramal yang kuat kepada pengulangan semula skizofrenia iaitu ia meningkatkan risiko sebanyak 8 kali ganda. Komponen KBK serta tret ekstraversi dan tret kebolehinginan sosial juga adalah peramal yang signifikan kepada pengulangan semula. Atribusi kestabilan bagi simptom positif yang dipegangi keluarga adalah faktor pengantara yang signifikan dalam perhubungan di antara: (i) simptom positif penghidap dengan indeks EE keluarga; (ii) simptom positif dengan komponen KBK keluarga; (iii) tret neurotikisme keluarga dengan indeks EE mereka; dan (iv) tret ekstraversi dan komponen KBK keluarga. Dapatan yang baharu oleh kajian ini adalah peranan tret ekstraversi dan kebolehinginan sosial keluarga yang secara langsung meramal pengulangan semula skizofrenia. Dua model yang mempunyai padanan yang baik dengan data kajian juga dibina bagi menerangkan tentang mekanisme yang mendasari pembentukan indeks EE dan komponen KBK serta proses pengulangan semula skizofrenia.,Previous studies conducted worldwide particularly within Western cultures have shown that negative interaction patterns in the form of high expressed emotion (high-EE), critical comments (high-CC) and emotionally over-involved (high-EOI) attitudes conveyed by relatives toward schizophrenia patients turned out to be robust psychosocial predictors of schizophrenia relapse. However, this phenomenon is not well understood in Malaysia. The present study aimed at investigating the: (i) profiles of patients’ symptomatology and relapse cases; (ii) profiles of relatives’ index EE as well as CC and EOI components; (iii) profiles of relatives’ personality traits and attributions held by them in explaining patients’ symptomatology; (iv) underlying mechanisms that contributed to the development of relatives’ high-EE index, high-CC and high-EOI attitudes; and (v) the underlying mechanisms of relapse processes. A total of 160 respondents (N=80 chronic schizophrenia patients and N=80 primary caregivers) participated in this study. Schizophrenia patients who were receiving inpatient psychiatric treatment, met the inclusion and exclusion criteria and were granted consents by their primary caregivers were interviewed in order to assess their symptomatology (Phase 1). The assessment was conducted via the Structured Clinical Interview for Positive and Negative Syndrome Scale (SCI-PANSS). These patients were followed-up 6 months later in order to check for possible relapse (Phase 2). Relatives’ expressed emotion (EE), personality traits and attributions were assessed by Family Questionnaire (FQ), Eysenck Personality Questionnaire-Revised short scale (EPQ-RS) and Attributions of Symptoms Inventory (ASI), respectively. Statistical analyses employed were one sample t-tests, independent sample t-tests, two-way contingency table analysis using crosstabs, Pearson Chi-square statistics, multiple regressions, logistic regressions and path analyses. Results revealed that patients’ positive and negative symptoms as well as their total symptomatology were statistically and clinically severe. About 52.5% of them had relapsed within 6 months. Relapse cases were significantly higher among patients living with high-EE and high-CC relatives. The severity of patients’ positive symptoms contributed significantly towards relatives’ high-CC attitudes, while relatives’ social desirability trait contributed significantly towards their high-EOI attitudes. Relatives’ extraversion and neuroticism traits as well as patients’ positive symptoms significantly predicted high-EE index. Relatives’ high-EE index predicted the risk of patients’ relapse to be 8 times higher. The CC component as well as extraversion and social desirability traits also turned out to be strong predictors of patients’ relapse processes. Relatives’ stability attribution regarding patients’ positive symptoms turned out to be significant mediators in the relationship between: (i) patients’ positive symptoms and relatives’ EE index; (ii) patients’ positive symptoms and relatives’ CC component; (iii) relatives’ neuroticism traits and their EE index; and (iv) relatives’ extraversion trait and their CC component. The novelty of the findings was demonstrated in the significant role of relatives’ extraversion and social desirability traits that directly predicted patients’ relapse. Two models which demonstrated goodness-of-fit to the data were also constructed to explain the underlying mechanisms of EE index and CC component developments and relapse processes.,PhD | - |
