dc.descriptionForward head posture (FHP) is associated with neck pain. Effectiveness of long term exercises in correcting FHP has been reported. However, prevalence of FHP remains unclear among adults with nonspecific neck pain (NSNP), more so in the Malaysian context. It is also not clear if three to six weeks of postural exercises are effective in reducing FHP among adults with NSNP. This study aimed to determine the prevalence of FHP in adults with NSNP and examine the effectiveness of three to six weeks of postural exercises in reducing FHP among NSNP. Prevalence of FHP was assessed among 167 participants (years: 25.2 + 8.67) with self-reported NSNP. FHP was assessed using photogrammetry method and FHP angles were measured using cervical angles (CA). Participants with FHP were randomized into two groups. The control group (n=15) did not receive any exercises and continued their usual care and the experimental group (n=15) received evidenced based home postural exercises program. FHP and other outcome measures which include neck range of motion (cervical range of motion device; CROM), hand grip strength (HGS) using hand dynamometer, pain (numeric pain rating scale; NRPS) and neck disability index (NDI) were assessed at baseline and re-assessed at three and six weeks post intervention. Prevalence of FHP was 65.3% among adults with NSNP. Analysis using mixed-design two-way repeated measure ANOVA demonstrated a significant interaction effect between time and group for CA, F(1.5, 40.7)=11.351,p< 0.001; right HSG, F(1.69, 45.74)=6.60, p<0.01; left HSG, F(1.27,34.31)=11.42, p<0.001; cervical flexion, F(2,54)=17.34, p<0.001; cervical extension, F( 1.63, 44.22)=32.33, p< 0.001; lateral flexion to right, F( 1.35, 36.48)=13.22, p<0.001; lateral flexion to left, F(1.39, 37.55)=16.18, p<0.001; left rotation, F(1.73, 46.92)=12.112, p< 0.001; NRPS, F (1.72, 48.18)=11.243, p<0.001.Greater mean percentage changes were shown in all measurements among the experimental group. Post-hoc analyses using Bonferroni method indicate greater improvement in all measurements from baseline to six weeks compared to three weeks post intervention. In conclusion, six weeks of home postural exercise program was effective in reducing FHP and pain; improving neck range of motions and muscle strength among adults with NSNP. Clinically, six weeks of neck postural exercises is required to correct FHP to more normal head postures.,Masters Of Health Science
