Title: Salvadora persica and its anti-plaque properties and benefits on periodontal health : a systematic review and meta-analysis

Description: Salvadora persica in its natural form and extract as toothpaste and mouthwash is one of the widely used alternatives for oral hygiene care to remove plaque and reduce gingival inflammation. Articles for this systematic review and meta-analysis was searched following the PRISMA Guideline, across five databases from 1st January 1970 until 31st December 2019. The total number of articles encountered were: MEDLINE (n = 1304), CENTRAL (n = 29), Science Direct (n = 276), PubMed (n = 176) and Google Scholars (n = 949), with additional records from other sources(Clinical Trial Register and Opengrey) (n = 9). A total of 35 articles found to fit the inclusion and exclusion criteria model, and qualitative synthesis was done. Among these, data from 24 studies were extracted and analysed in the quantitative synthesis. 24 trials with 1729 samples focusing on anti-plaque function, showed that usage of control arm (either toothbrush, non-herbal toothpaste or chlorhexidine mouthwash) showed a better outcome with effect size 0.44[-0.66,0.94] (p = 0.20). Subsequently, 13 trials with 624 samples found that Salvadora persica is better as anti-gingivitis function with effect size -0.31[-0.84,0.22] (p = 0.25). Subgrouping analyses based on different forms indicated the chewing stick is better as anti-plaque with effect size - 0.07[-0.60,0.45] (p = 0.78), as well as anti-gingivitis with effect size -2.07[-4.05, - 0.10] (p = 0.04). Salvadora persica toothpaste is also better when compared to nonherbal toothpaste as anti-plaque, with effect size -0.39[-0.80,0.03] (p = 0.07) as well as anti-gingivitis, with effect size -0.42 [-1.66,0.82] (p = 0.51). Salvadora persica mouthwash shows an inferior effect compared to chlorhexidine for both function; as anti-plaque with effect size 0.93[0.03,1.84] (p = 0.04) and anti-gingivitis with effect size 0.03[-0.59,0.66] (p = 0.92). As a conclusion, usage of Salvadora persica chewing sticks and toothpaste is better compared to the standard toothbrush and non-herbal toothpaste, as anti-plaque and anti-gingivitis function and overall periodontal health. Based on this study, the usage of Salvadora persica in its natural form and toothpaste is considered effective and may be used as an alternative for dental care.,Ijazah Doktor Pergigian Klinikal (Periodontologi)
