Description: | Mild cognitive impairment (MCI) is an etiologically heterogeneous syndrome characterized by memory performance below the age norm and represents a transitional state between normal aging and dementia disorders. Ulam or traditional vegetable is commonly consumed by the Malay and specific Asian population and has been reported to be rich in polyphenols and antioxidants. Therefore, this study aimed to explore the relationship between ulam consumption and cognitive function and develop Cosmos caudatus (CC) supplement to investigate the efficacy of CC supplement on neurodegenerative prevention among older adults with MCI. This study consisted of three phases. Phase 1 was a cross-sectional study involving 252 Malay aging adults from low socioeconomic households to determine the type and frequency of habitual ulam consumption and its relationship with the subjects' cognitive function and brain activation. Phase 2 was to develop CC supplement and analyse its nutrition composition and bioactive compound. Phase 3 was a double-blind, placebo-controlled clinical trial conducted for 12 weeks with 24 older adults with MCI in each group. This control trial was to investigate the efficacy of CC supplement in improving the cognitive function, mood status, brain activation, cognitive-related biomarkers and other health parameters. Findings of Phase 1 indicated that CC surpassed the list among all habitual ulam intake by the subjects. After adjusting for confounding factors, habitual ulam consumption related to better Mini-mental state examination (MMSE) scores (β=0.029, p=0.009), Digit Span (β=0.047, p<0.001), Rey Auditory Verbal Learning Test (RAVLT) immediate recall scores (β=0.113, p=0.010), right dorsolateral prefrontal cortex (DLPFC) activation for 0-back (β=0.007, p=0.024) and 1-back (β=0.004, p=0.047) and left DLPFC for Stroop Colour Word Test (SCWT-1) (β=0.047, p=0.042) and SCWT-2 (β=0.015, p=0.038). Further ANCOVA analysis also reported that consumption of ulam at 100th percentile (≥30 g/day) associated with better cognitive function and brain activation as compared to 25th and 50th percentile (p<0.05) after adjustments for confounding factors for all relevant parameters. Based on Phase 2, CC supplements were successfully developed and formulated which contain higher quercetin and quercitrin content as compared to Polygonum minus aqueous extract. It was found that approximately 95 g (six servings or three cups) of CC on a wet basis could yield 500 mg of CC supplement. In Phase 3, a two-way mixed ANOVA analysis showed significant intervention effect on MMSE scores (partial η2=0.150, p=0.049), SCWT (Congruent) correct response (partial η2=0.439, p=0.019), right DLPFC activation while performing 1-back task (partial η2=0.396, p=0.016) and left DLPFC while performing SCWT (Congruent) task (partial η2=0.285, p=0.049) and significant decrement in tension (partial η2=0.191, p=0.018), total mood disturbance (partial η2=0.171, p=0.028) and serum MDA (partial η2=0.097, p=0.047). Furthermore, there was significant increment in percentage mean change of Digit Span and serum glutathione level in the CC supplement group as compared to the placebo group (p<0.05). In conclusion, CC supplement has the potential to act as mild cognitive impairment preventive agent by reducing lipid peroxidation, increasing serum glutathione level and improving DLPFC activation leading to improve global cognitive function and working memory, reduced tension and total mood disturbance, among older adults with MCI after 12 weeks,Doktor Falsafah(PhD) |
