Description: Introduction and problem statement: In order to minimise relapse and maintain arch form, several retention appliances have been used after orthodontic treatment. To date, Hawley retainers (HRs) are more commonly used in orthodontic retention where expanded arch stabilization is needed. However, researchers have paid little attention to relapse in expansion cases. Most published studies compare the arch width, as well as incisor irregularity in evaluating relapse in different groups of retainers. There is a notable paucity of a high-quality study comparing the effectiveness of HRs and modified vacuum-formed retainers (mVFRs) in expansion cases. Hence, this study was proposed. The modified retainer being investigated in this study would be made as efficient and as quick as the normal VFRs, which would cost less in terms of labour, materials and quicker to fabricate. Aim: This study aimed to compare the clinical effectiveness of HRs and mVFRs with palatal coverage in maintaining transverse expansion over a 6-month period. Methods: The present study is a multicentre randomised controlled trial (RCT). Post-orthodontic treatment patients from the Orthodontic Unit of UKM, Klinik Pakar Ortodontik Klinik Kesihatan Bandar Botanik Klang and Unit Pakar Ortodontik Klinik Pergigian Sungai Chua Kajang, who met the inclusion criteria were invited to participate. Thirty-five subjects were randomly allocated into two groups: HR (17 subjects, age: 21.88 ± 4.12, gender: 5 males and 12 females) and mVFR (18 subjects, age: 22.06 ± 5.88, gender: 7 males and 11 females). Dental casts of subjects were assessed at debond (T0), 3-month (T1) and 6-month (T2) post-debond. Intercanine width (ICW), interpremolar width (IPMW), first molar intermesiobuccal cusp (FIMBCW) and first molar interdistobuccal cusp (FIDBCW) were compared between groups over time by using Mixed ANOVA. Results: No statistically significant differences were found between the two retainer groups at ICW (P=0.60), IPMW (P=0.26), FIMBCW (P=0.06) and FIDBCW (P=0.24) during the 6-month retention period.
