dc.description | PUFA Index was introduced to detect the clinical consequences of untreated dental caries. To date, there are no studies on the reliability and accuracy of PUFA index as a screening tool, limiting its usage in large scale prevalence studies. The aim of this study was to evaluate the diagnostic accuracy of PUFA index in comparison to Periapical Index (PAI) and gold standard clinical diagnosis when used to screen for endodontic disease among Malaysian subpopulation. The prevalence of caries and endodontic disease were also assessed. A total of 165 Malaysian adults who attended the Primary Care Unit at the Faculty of Dentistry, Universiti Kebangsaan Malaysia, Kuala Lumpur were screened for caries according to DMFT Index. The clinical consequences of untreated dental caries were screened by the presence of Pulpal involvement (P), Ulceration(U), Fistula (F) and Abscess (A) using PUFA Index. Orthopantomographs (OPG) were used to assess the periapical status whereby all teeth were scored based on the Periapical Index (PAI). The gold standard clinical diagnosis of all teeth was derived based on the American Association of Endodontists 2013 diagnosis classification system. Prevalence of DMFT > 0 was 100% (n=165) with mean index of 10.90. Prevalence of PUFA >0 was 98.2% (n=162). Mean number of teeth affected (PUFA) was 2.72. Out of 4115 teeth assessed, 16.2% (n=666) were diagnosed with pulpal infection, 16.2% (n=325) with periapical infection. The intraand inter-examiner reliability of PUFA in screening endodontic disease was high at 0.76, 0.83 and 0.87 respectively. Intra- and inter-examiner reliability of PAI in screening for endodontic disease was also high at 0.72, 0.75 and 0.80 respectively. The sensitivity of PUFA Index was moderate (0.68, 0.88) but the specificity was high (0.99, 0.95) when compared to gold standard pulpal and periapical diagnosis respectively. Sensitivity of PAI Index was lower (0.42,0.75) but the specificity was high (0.99,.98) when compared to pulpal and periapical disease respectively. Using the Receiver Operating Characteristics Estimation test, PUFA was more accurate than PAI in screening endodontic disease (p<0.0001). Results suggest that there was a high caries experience (DMFT) and moderate prevalence of endodontic disease (PUFA) among this Malaysian subpopulation. PUFA is more reliable and accurate in screening for endodontic disease compared to PAI.
